We embrace each hospice patient with care, comfort and dignity. Our Hospice Team relieves the patient and family of the time-consuming task of coordinating care and other related services, allowing the patient and family to spend more time with each other.
Patient care is our concern.
The Primary Nurse relieves the patient and his or her family of the time-consuming task of coordinating care and other related services, allowing the family to spend more time with each other. The Primary Nurse also knows the patients’ rights when it comes to hospice coverage from Medicare, Medicaid, and private insurers, ensuring that equipment, supplies, medications and other services are provided.
We go wherever our patients are.
The Hospice Team also ensures that hospice care is provided whether the patient is residing at home, a nursing home, assisted living facility or even a hospital. We work with the primary care physician or medical director to manage pain and symptoms related to the patient’s medical condition.
Bridge Program/Palliative Care
Nursing Placement’s Bridge Program offers clients with a newly diagnosed terminal illness, and still receiving treatment, the opportunity to receive skilled nursing service from our Home Care Team. Once the client, family and primary care physician decide there will be no further treatment and the disease follows its usual course of progression, the client may transition to our Hospice program. The team caring for the client will remain the same, assuring continuity of care.
